Begin an Admission or Referral

This is Rogers Memorial Hospital’s admissions information page for Chemical Dependency Services. IF YOUR LIFE IS IN DANGER, CALL 911.

Getting Started: In most cases, an initial telephone screening starts the admitting process.

Call us: To begin your admission or referral, call (800) 767-4411. You may also use our online form to request a free assessment.

Medical Review: The information you share with one of our admissions specialists during this phone call will be reviewed with one of our expert physicians, who will recommend an appropriate level of care.

Our proven treatment is offered at a lower cost when compared to many other programs. While other providers often have no choice but to keep their patients in high-cost inpatient settings after stabilization, Rogers Memorial’s progressive approach to residential treatment allows us to provide high quality treatment. Rates at our intensive residential treatment centers are less than half the daily cost of most inpatient care, and you have the peace of mind knowing that, should they become necessary, on-site inpatient services are immediately available.

Next step: After the review, admissions staff will contact you and share the physician’s recommendation.
