  • Medical Director, Child and Adolescent Eating Disorder Programs
    Rogers Memorial Hospital-Milwaukee

    Dr. Tracey Cornella-CarlsonTracey Cornella-Carlson, MD, a board-certified child, adolescent and adult psychiatrist, is the medical director of the child and adolescent inpatient and the partial hospitalization eating disorder programs at Rogers Memorial Hospital – Milwaukee. She received her medical degree from the Chicago Medical School in 1992. At Rush Presbyterian St. Luke's Medical Center in Chicago, Dr. Cornella-Carlson completed a three-year general psychiatry residency and a two-year fellowship in child and adolescent psychiatry, during which she was Chief Fellow. She has done research on co-morbidity in boys with Attention Deficit/Hyperactivity Disorder, and specializes in eating disorders.

    Dr. Cornella-Carlson is licensed to practice psychiatry in the state of Wisconsin, and a current member of the Academy for Eating Disorders and the American Academy of Child and Adolescent Psychiatry.

    Dr. Stephanie EkenStephanie C. Eken, MD, a child and adolescent psychiatrist, earned her medical degree from the University of Tennessee College of Medicine in 2002 and her bachelor's degree from the University of Richmond. She served as chief resident during her triple board residency training in pediatrics, psychiatry, and child and adolescent psychiatry at the University of Kentucky.


    Dr. Eken received the "Catcher in the Rye Award for Advocacy" from the American Academy of Child and Adolescent Psychiatry in 2008 and has received numerous other honors and fellowships. She has both authored and presented to professional and academic audiences. Her clinical interests include depression, anxiety, obsessive-compulsive disorder, attention deficit hyperactivity disorder, psychotic disorders, and emotional problems in chronically medically ill children.

    Dr. HaileyJennifer L. Hailey, DO is an adult psychiatrist who specializes in the treatment of eating disorders and addictions.
    She is a member of the Michigan Osteopathic Association, American Osteopathic Association, Michigan Psychiatric Society, American Psychiatric Association and the Michigan Psychoanalytic Institute.
    Dr. Hailey was an active participant in the Dialectical Behavior Therapy group at The DBT Center of Michigan. She completed her psychiatry residency at Michigan State University and her osteopathic internship at Botsford Hospital, Farmington Hills, Mich. She is a graduate of Nova Southeastern University, Ft. Lauderdale.

    Dr. Stephanie EkenJerry L. Halverson, MD, is a board-certified adult psychiatrist with a subspecialty in psychosomatic medicine. He serves as medical director for adult services at Rogers Memorial Hospital - Oconomowoc.

    Dr. Halverson has been honored by his colleagues for clinical and leadership excellence. He has been selected for a 2010 "Forward under Forty" award by the University of Wisconsin Alumni Association and has also been awarded an American Medical Association Foundation 2010 leadership award for outstanding community and peer leadership as an early career physician.

    Dr. Halverson is a recognized leader within the Wisconsin Medical Society and the Wisconsin Psychiatric Association and has held key leadership positions within professional organizations both locally and nationally. He has spoken extensively on mental health disorders and advocacy efforts to academic, legislative and community audiences.

    Prior to joining Rogers Memorial, Dr. Halverson was the medical director of adult psychiatry at Meriter Hospital in Madison and was an assistant professor of psychiatry at the University of Wisconsin School of Medicine and Public Health where he directed the Treatment Refractory Psychiatric Disorders Program.

    Dr. Jay KasnerJay Kasner, MD is the medical director of Chemical Dependency Services at Rogers Memorial Hospital – Milwaukee and the attending physician at The Herrington Recovery Center. Dr. Kasner is board certified as an addictionologist by the American Society of Addiction Medicine (ASAM). He is respected throughout the medical community for his clinical expertise with inpatient, residential and outpatient treatment programs.

    Prior to joining Rogers Memorial, Dr. Kasner served as director of chemical dependency services at Charter Behavioral Health in Milwaukee and as medical director of Kettle Moraine Hospital in Oconomowoc. For more than 25 years, Dr. Kasner has limited his medical practice to treating people who are chemically dependent. This focus gives him a unique understanding of the pressures many of our patients face as they learn how to overcome the disease of addiction and live more productive lives.

    Dr. Vineet KulkarniDr. Kulkarni, the medical director of Adult Services of Rogers Memorial Hospital–Milwaukee, is a board certified psychiatrist, and specializes in the treatment of adults and adolescents. He specializes in the treatment of attention deficit hyperactivity disorder (ADHD), affective disorders and anxiety disorders. Dr. Kulkarni received his MBBS (our equivalent of M.D.) at T.N. Medical College-Bombay India and completed residency at the University of Illinois Medical College of Psychiatry.

    Dr. Peter LakeDr. Lake is a board-certified child, adolescent, and adult psychiatrist and is the medical director of Rogers Memorial Hospital-Oconomowoc, Child and Adolescent Services-Oconomowoc and the Child & Adolescent Center.

    Dr. Lake has been in private practice since 1991, serving as vice chairman in the Department of Psychiatry at Physicians Plus Medical Group, S.C.; and associate medical director for Physicians Plus Mental Health Case Management and Consultation System, Madison until fall of 1999. He was also an associate clinical professor at the University of Wisconsin Medical School. Currently, Dr. Lake practices full time at Rogers Memorial.

    Dr. Lake's clinical area of interest includes early childhood development, attachment disorders, attention deficit hyperactivity disorder (ADHD), childhood depression and anxiety disorders, medication management, and individual psychotherapy and family therapy. He is a frequent lecturer and consultant to schools, social service agencies, and case management systems. Additionally, Dr. Lake is a co-founder of Challenge Camp and a board member of the Rogers Memorial Hospital Foundation, a charitable organization for patients needing financial assistance to complete treatment.

    Dr. Lake received his medical degree from the University of Iowa. He completed his residency in adult psychiatry and fellowship in child and adolescent psychiatry at Vanderbilt University in Nashville, Tennessee. He was the recipient of the 1989 Marc C. Hollender M.D. Award for Outstanding Performance by a Psychiatric Resident, Vanderbilt University, Department of Psychiatry; and 1982 Superior Academic Achievement Award, Creighton University in Omaha, Nebraska, where he completed his undergraduate degree.

  • Dr. Tim LevenhagenDr. Levenhagen is board certified in adult psychiatry and is a full-time attending psychiatrist with the acute adult inpatient unit at Rogers Memorial Hospital – Oconomowoc. Mood and anxiety disorders, as well as the geriatric population are his major areas of clinical interest.

    Dr. Levenhagen received his undergraduate and medical degrees from the University of Wisconsin – Madison. While completing his residency program in psychiatry at the Medical College of Wisconsin, Dr. Levenhagen was the chief resident of psychiatry at the VA Hospital in Milwaukee, Wisconsin.

  • Dr. Roger LuhnDr. Luhn is a board-certified child and adolescent psychiatrist and a full time medical staff member of the Child & Adolescent Center at Rogers Memorial Hospital in Oconomowoc. Prior to joining Rogers Memorial, Dr. Luhn was a medical director at the Mercy Cooperative Childcare Institute in Janesville, and a Mendota Mental Health Institute child and adolescent psychiatrist in Madison. He has directed inpatient, outpatient and residential programs for children and adolescents.
    Dr. Luhn earned an undergraduate degree in biology at Whitworth College in Spokane, Washington. He earned his medical degree at the University of California in San Diego, California. Dr. Luhn is a member of the American Academy of Child and Adolescent Psychiatry.

    Dr. Michael M. MillerDr. Miller has practiced Addiction Medicine for almost 30 years and is certified in Addiction Medicine by the American Board of Addiction Medicine. He is also a board-certified general psychiatrist and addiction psychiatrist. He is Fellow of the American Psychiatric Association (APA) and the American Society of Addiction Medicine (ASAM).

    Dr. Miller is also a Director of the American Board of Addiction Medicine. He recently completed a two-year term as the president of ASAM, the largest medical specialty society in America of physicians devoted to addiction prevention, treatment, medical education, research, and public policy. He has served on many task forces and councils of ASAM and the Wisconsin Medical Society (WMS), was chair of the WMS Commission on Addictive Diseases, and is current Vice Speaker of the WMS House of Delegates. For over 10 years he served with Dr. Roland Herrington on the Managing Committee of the Statewide Physician Health Program of the WMS.

    Prior to joining Rogers Memorial, Dr. Miller was the Associate Director for the Addiction Psychiatry Fellowship at the University of Wisconsin School of Medicine and Public Health and Wm. S. Middleton Veterans Administration Hospital. He continues to serve as Associate Clinical Professor at the UW School of Medicine and Public Health in the Departments of Psychiatry, Family Medicine, and Internal Medicine. His previous professional positions include the Midelfort Clinic and Luther Hospital in Eau Claire, WI, and over 20 years at Meriter Hospital in Madison, WI in its NewStart Program.

    Dr. Miller has lectured throughout the United States and internationally on addiction medicine, general psychiatry, and addiction psychiatry topics, as well as on topics related to medical economics, medical quality, medical ethics, and public health.

  • Dr. Carol MullenDr. Mullen is a full-time psychiatrist and addictionologist in adult services at Rogers Memorial Hospital-Milwaukee. Previously, she was a staff psychiatrist at St. Michael's Hospital in Milwaukee. Dr. Mullen began her medical career as a registered nurse, and then completed a three-year internal medicine residency and a three-year psychiatric residency at Sinai Samaritan Medical Center in Milwaukee. She is certified in addiction medicine through the American Society of Addiction Medicine and is experienced in treating patients dealing with dual diagnosis as well as general adult psychiatric concerns.

    Khazi A. MuqeetKhazi A. Muqeet, MD is a board-certified adult psychiatrist at Rogers Memorial Hospital – Milwaukee who specializes in the treatment of major mental illnesses and addiction.

    Dr. Muqeet has trained extensively in the treatment psychiatric disorders in the United States, Great Britain and India. He completed a fellowship in forensic psychiatry at the Medical College of Wisconsin where he also completed his psychiatry residency and served as chief resident. He completed an additional residency in psychiatry in the North Wales Psychiatry residency program where he also served as chief resident.
    In addition to serving in leadership positions during his training, Dr. Muqeet was the recipient of several awards and recognition for his academic excellence. He has presented to numerous clinical and academic audiences internationally. He has a research interest in the applicability of genetic testing in psychiatric disorders.

    Dr. Jody PahlavanDr. Pahlavan is a clinical psychologist licensed by the State of Wisconsin and is the program director of the Child & Adolescent Services Day Treatment Program of Rogers Memorial Hospital–Milwaukee. Dr. Pahlavan has been in private practice since 1995 and has more than 15 years of experience working with children and adolescents.

    Dr. Pahlavan received her Doctor of Psychology degree from the Wisconsin School of Professional Psychology. She completed her residency in child psychology at Children’s Hospital of Wisconsin. She completed her master’s degree in psychology from Marquette University and her bachelor’s degree in special education from Cardinal Stritch College.

    Dr. Pahlavan has given numerous talks on behavioral health issues affecting youth. She is a member of the American Play Therapy Association, the American Psychological Association, the Wisconsin Psychological Association, Children and Adults with Attention-Deficit/Hyperactivity Disorder (CHADD), and is listed with the National Register of Health Care Providers.

    Dr. Kambiz PahlavanDr. Pahlavan is the medical director of the Rogers Memorial Hospital - Milwaukee and Child & Adolescent Services of Milwaukee. His primary objective is to continue Rogers Memorial's tradition of excellence in identifying the mental health needs of the community and developing high quality services within the hospital, as well as promoting education and research.

    Dr. Pahlavan has worked at McLean Hospital, a teaching hospital of Harvard Medical School, where he clinically supervised child psychiatry fellows and general psychiatry residents, and presented weekly seminars on child psychiatry. Since 1993, Dr. Pahlavan worked with the Medical College of Wisconsin as the clinical supervisor for medical students and several general psychiatry residents. With more than 25 years of psychiatry experience, he has developed numerous lectures including adolescent suicide, attention deficit hyperactivity disorder (ADHD), pervasive developmental disabilities, autism (including spectrum disorders) and child and adolescent affective disorders. He has presented papers at the American Psychiatric Association and the AACAP. He was the 1999 recipient of the American Psychiatric Association’s Nancy C.A. Roeske Certificate of Excellence in Medical Student Teaching.

    Dr. Pahlavan is board certified through the American Board of Psychiatry and Neurology and the American Board of Child Psychiatry. He completed his general psychiatry residencies at Norwich Hospital, Connecticut; Medfield State Hospital, Massachusetts; and New England Medical Center in Boston. He completed his fellowship in child psychiatry at McLean Hospital, Harvard Medical School in Massachusetts. He is a fellow of American Psychiatric Association and the American Academy of Child & Adolescent Psychiatry.

  • Clinical Director, The Obsessive-Compulsive Disorder Center and Cognitive-Behavioral Therapy Services

    Dr. Brad RiemannDr. Riemann is a leading expert in the assessment of anxiety disorders and use of cognitive-behavioral therapy (CBT) treatment. He supervises the training of graduate and post-graduate students from around the country for CBT in anxiety disorders and collaborates with colleges and universities on research projects investigating obsessive-compulsive disorder (OCD) and other anxiety disorders.

    Dr. Riemann serves as chairman for the clinical advisory committee of the International OCD Foundation (IOCDF) and serves on its scientific advisory board. He also serves on the clinical advisory board for the Anxiety Disorders Association of America (ADAA).

    He has authored numerous scientific papers on obsessive-compulsive disorder and anxiety and has spoken at national and international conventions, including the Association for Behavioral and Cognitive Therapies, the ADAA and the IOCDF. Dr. Riemann has also been featured on the national television shows 48 Hours and The Today Show.

    Dr. Riemann received his doctorate in clinical psychology from the Chicago Medical School. He is also a clinical assistant professor in the department of psychology at the Rosalind Franklin School of Medicine, Marquette University, and the University of Wisconsin-Milwaukee.

    Alexander M. Scharko, MD nAlexander M. Scharko, MD is a triple board-certified psychiatrist who specializes in the treatment of children and adolescents. He is board certified by the American Board of Psychiatry and Neurology in general psychiatry, child and adolescent psychiatry and psychosomatic medicine.

    Dr. Scharko is a member of the American Psychiatric Association, the American Academy of Child and Adolescent Psychiatry, and the Academy of Psychosomatic Medicine. He is a clinical faculty member in the Division of Child and Adolescent Psychiatry at the Medical College of Wisconsin.

    His research interests include pediatric behavior dysregulation disorders, mood disorders, psychotic disorders and psychiatric medication adverse effects in children and adolescents.

    Dr. Scharko is a graduate of the University of Wisconsin-Madison School of Medicine and Public Health. His postdoctoral work included a research fellowship in the Department of Psychiatry and Behavioral Sciences at Johns Hopkins University School of Medicine in Baltimore. He completed his residency in psychiatry along with his clinical fellowship in child and adolescent psychiatry at the University of Wisconsin-Madison School of Medicine and Public Health.

    He has numerous awards, honors, and publications and frequently gives presentations to professional and academic audiences on the topic of child and adolescent psychiatry.

  • Ulland, MDErik Johnston Ulland is a 2003 graduate of Loma Linda University School of Medicine and performed residency at the University of Wisconsin Hospital and Clinic's Child Psychiatry program from 2006 to 2008, and at Dartmouth-Hitchcock Medical Center in Lebanon, N.H. from 2003 to 2006.

    Dr. Ted WeltzinTheodore E. Weltzin, MD is a board-certified adult psychiatrist who is an established leader in the field of eating disorders.

    He serves on the board of directors for the Eating Disorder Coalition, is a member of the American Psychiatric Association and Academy of Eating Disorders and an Assistant Clinical Professor of Psychiatry at the Medical College of Wisconsin.

    Dr. Weltzin has contributed to the research and treatment of eating disorders with more than 45 peer-reviewed publications, invited book chapters and international lectures. He was a reviewer and consultant for the first American Psychiatric Association “Practice Guidelines for Eating Disorders” published in 1993 and is an Editorial Board Member for the journal “Eating Disorders: The Journal of Treatment & Prevention.”

    Dr. Weltzin has appeared as an expert on CNN, CBS Morning News, Good Morning America, The View and The Dr. Phil Show.

    As medical director of Eating Disorder Services at Rogers Memorial Hospital since 2001, Dr. Weltzin has provided leadership in all areas of development and clinical oversight of one of the largest eating disorder treatment centers in the country.

    He has trained well over 100 clinicians in the field and developed innovative programs addressing the unique needs of eating disorder patients. These programs include residential, inpatient and partial hospital treatment as well as programs for patients with eating disorders and anxiety, excessive exercise and males with eating disorders.

  • Dr. Azhar M. YunusAzhar M. Yunus, M.D., is a full-time physician for Child and Adolescent services at our Milwaukee hospital. Yunus is a 1999 graduate of Saba University of School of Medicine in the Netherlands, and performed his residency at the University of Wisconsin Hospital and Clinics from 2004 to 2008.
