Andrew Bennett, M.D.
Eating Disorder Services
Andrew Bennett, MD, a board-certified psychiatrist, is now a full-time member of the Rogers Memorial eating disorder medical staff. He most recently served as Unit Chief of the Eating Disorder Program at New York-Presbyterian Hospital. Dr. Bennett received his medical degree from Columbia University and did his residency at the Payne Whitney Clinic at Cornell University. Bennett was a research fellow at the Eating Disorder Research Institute at New York State Psychiatric Institute.
Tracey Lee Cornella-Carlson, M.D.
Eating Disorder Services
Dr. Cornella-Carlson is a board-certified child, adolescent, and adult psychiatrist in the eating disorder services at Rogers Memorial Hospital in Oconomowoc, WI. She received her medical degree from the Chicago Medical School in 1992. At Rush Presbyterian St. Luke's Medical Center in Chicago, Dr. Cornella-Carlson completed a three-year general psychiatry residency and a two-year fellowship in child and adolescent psychiatry, during which she was Chief Fellow. She has done research on co-morbidity in boys with Attention Deficit/Hyperactivity Disorder, and specializes in eating disorders.
Dr. Cornella-Carlson is licensed to practice psychiatry in the state of Wisconsin, and a current member of the Academy for Eating Disorders and the American Academy of Child and Adolescent Psychiatry.
Charles Engel, MD
Medical Director, The Herrington Recovery Center
Dr. Engel is board certified in addiction medicine, and has worked in the field since 1981. He provided consultation to various health insurers in the state of Wisconsin regarding substance abuse treatment guidelines, and was the first medical director of the St. Anthony Hospital AODA program and the St. Michael Hospital chemical dependency service. Dr. Engel is assistant clinical professor of psychiatry at the Medical College of Wisconsin, associate medical director of the Wisconsin State Medical Society Physician Health Program and has a private office practice.
Michael Goldstone, M.D.
Medical Director, The Herrington Recovery Center
Dr. Goldstone is the medical director of The Herrington Recovery Center, a Rogers Memorial Hospital residential program for overcoming addictions. He is board certified in internal medicine and certified in addiction medicine. Dr. Goldstone also is an assistant clinical professor of medicine at the Medical College of Wisconsin and has a private office practice.
Dr. Goldstone graduated from St. Louis University School of Medicine and completed his residency and fellowship at Medical College of Wisconsin affiliated hospitals.
Lauri Green, M.D.
Medical Director, The Child Center
Lauri Green, MD, a board-certified child and adolescent and adult psychiatrist valued by Milwaukee and Waukesha families for patient-focused care, has joined the Rogers Memorial medical staff in Oconomowoc. She is Medical Director of The Child Center, a new residential program serving children ages 8 to 12. In private practice in the Milwaukee area since 2000, Dr. Green is a graduate of the University of Wisconsin-Milwaukee and earned her medical degree at the Medical College of Wisconsin. She is a member of The American Academy of Child and Adolescent Psychiatry and the Wisconsin Medical Society.
Harold H. Harsch, M.D.
Medical Director, Medical College Psychiatric Services
Dr. Harsch, a board-certified adult and geropsychiatrist, is the Medical Director of the Medical College Psychiatric Service at Rogers Memorial Hospital. At the Medical College, he holds appointments as associate professor in psychiatry, behavioral medicine, and medicine. Dr. Harsch's research has focused on the development of new antidepressant and antipsychotic agents, and on the use of brain imaging to study such disorders as panic, dementia and schizophrenia. Dr. Harsch received is B.S. degree from Marquette University, and his M.D. degree from the Medical College of Wisconsin. He completed his residency and fellowship in adult psychiatry and behavioral medicine from Stanford University. Among Dr. Harsch's honors are the Physician's Recognition Award by the American Medical Association, and the Excellence in Teaching Award from the Medical College.
Jay Kasner, MD

Dr. Kasner is board certified through the American Society of Addiction Medicine, and is the medical director of the Adult Chemical Dependency Partial Hospitalization Program of Rogers Memorial Hospital-Milwaukee. Dr. Kasner has limited his practice to the treatment of chemically dependent individuals for the past 15 years – he was most previously director of chemical dependency services at Charter Behavioral Health from 1989-2000; and was medical director of Kettle Moraine Hospital, Oconomowoc, from 1986-89.
Vineet Kulkarni, MD.

Dr. Kulkarni, the medical director of Adult Services of Rogers Memorial Hospital–Milwaukee, is a board certified psychiatrist, and specializes in the treatment of adults and adolescents. He specializes in the treatment of attention deficit hyperactivity disorder (ADHD), affective disorders and anxiety disorders. Dr. Kulkarni received his MBBS (our equivalent of M.D.) at T.N. Medical College-Bombay India and completed residency at the University of Illinois Medical College of Psychiatry.

Dr. Lake is a board-certified child, adolescent, and adult psychiatrist and is the medical director of Rogers Memorial Hospital-Oconomowoc, Child and Adolescent Services- Oconomowoc and the Child & Adolescent Center. Dr. Lake has been in private practice since 1991, serving as vice chairman in the Department of Psychiatry at Physicians Plus Medical Group, S.C.; and associate medical director for Physicians Plus Mental Health Case Management and Consultation System, Madison until fall of 1999. He was also an associate clinical professor at the University of Wisconsin Medical School. Currently, Dr. Lake practices full time at Rogers Memorial.
Dr. Lake’s clinical area of interest includes early childhood development, attachment disorders, attention deficit hyperactivity disorder (ADHD), childhood depression and anxiety disorders, medication management, and individual psychotherapy and family therapy. He is a frequent lecturer and consultant to schools, social service agencies, and case management systems. Additionally, Dr. Lake is a co-founder of Challenge Camp and a board member of the Rogers Memorial Hospital Foundation, a charitable organization for patients needing financial assistance to complete treatment.
Dr. Lake received his medical degree from the University of Iowa. He completed his residency in adult psychiatry and fellowship in child and adolescent psychiatry at Vanderbilt University in Nashville, Tennessee. He was the recipient of the 1989 Marc C. Hollender M.D. Award for Outstanding Performance by a Psychiatric Resident, Vanderbilt University, Department of Psychiatry; and 1982 Superior Academic Achievement Award, Creighton University in Omaha, Nebraska, where he completed his undergraduate degree.

Dr. Levenhagen, the Medical Director of the Adult & Older Adult Services at Rogers Memorial’s Oconomowoc campus, is board-certified in adult psychiatry through the American Board of Psychiatry. Mood and anxiety disorders, as well as the geriatric population are his major areas of clinical interest. Dr. Levenhagen maintains a private practice and provides nursing home consultations in southeastern Wisconsin. He delivers lectures, seminars, presentations, and trainings to doctors, residents, and other professionals on numerous mental health topics.
Roger Luhn, MD

Dr. Luhn is a board-certified child and adolescent psychiatrist and a full time medical staff member of the Child & Adolescent Center at Rogers Memorial Hospital in Oconomowoc. Prior to joining Rogers Memorial, Dr. Luhn was a medical director at the Mercy Cooperative Childcare Institute in Janesville, and a Mendota Mental Health Institute child and adolescent psychiatrist in Madison. He has directed inpatient, outpatient and residential programs for children and adolescents.
Dr. Luhn earned an undergraduate degree in biology at Whitworth College in Spokane, Washington. He earned his medical degree at the University of California in San Diego, California. Dr. Luhn is a member of the American Academy of Child and Adolescent Psychiatry.
Elpidio Abanilla Mariano, MD
Adolescent and Adult Services : RMH Oconomowoc
Dr. Mariano is a full time psychiatrist at the Rogers Memorial Hospital Oconomowoc campus. He treats adult patients as well as adolescents age 13 and older, and has a particular interest in young adults with a variety of mood and anxiety disorders.
Born in Milwaukee, Dr. Mariano grew up in Eastern Iowa, and received his medical degree from the University of Iowa College of Medicine in Iowa City, Iowa where he was also inducted into the Gold Humanism Honor Society. Dr. Mariano completed his residency at the University of Washington in Seattle, Washington, where he was Chief Resident of Outpatient Services and Consult-Liaison. Dr. Mariano is a member of the American Psychiatric Association and the Wisconsin Medical Society.
Carol J. Mullen
DO Adult Services : RMH Milwaukee
Dr. Mullen is a full-time psychiatrist and addictionologist in adult services at Rogers Memorial Hospital-Milwaukee. Previously, she was a staff psychiatrist at St. Michael's Hospital in Milwaukee. Dr. Mullen began her medical career as a registered nurse, and then completed a three-year internal medicine residency and a three-year psychiatric residency at Sinai Samaritan Medical Center in Milwaukee. She is certified in addiction medicine through the American Society of Addiction Medicine and is experienced in treating patients dealing with dual diagnosis as well as general adult psychiatric concerns.
Jody A. Pahlavan, PsyD
Director, Child & Adolescent Day Treatment Services : RMH Milwaukee
Dr. Pahlavan is a clinical psychologist licensed by the State of Wisconsin and is the program director of the Child & Adolescent Services Day Treatment Program of Rogers Memorial Hospital–Milwaukee. Dr. Pahlavan has been in private practice since 1995 and has over fifteen years of experience working with children and adolescents.
Dr. Pahlavan received her Doctor of Psychology degree from the Wisconsin School of Professional Psychology. She completed her residency in child psychology at Children’s Hospital of Wisconsin. She completed her master’s degree in psychology from Marquette University and her bachelor’s degree in special education from Cardinal Stritch College.
Dr. Pahlavan has given numerous talks on behavioral health issues affecting youth. She is a member of the American Play Therapy Association, the American Psychological Association, the Wisconsin Psychological Association, Children and Adults with Attention-Deficit/Hyperactivity Disorder (CHADD), and is listed with the National Register of Health Care Providers.
Kambiz Pahlavan, MD, FAPA, FAACAP

Dr. Pahlavan is the medical director of the Rogers Memorial Hospital-Milwaukee campus and the Child & Adolescent Services of Milwaukee. His primary objective is to continue Rogers Memorial’s tradition of excellence in identifying the mental health needs of the community and developing high quality services within the hospital, as well as promoting education and research.
Dr. Pahlavan has worked at McLean Hospital, a teaching hospital of Harvard Medical School, where he clinically supervised child psychiatry fellows and general psychiatry residents, and presented weekly seminars on child psychiatry. Since 1993, Dr. Pahlavan worked with the Medical College of Wisconsin as the clinical supervisor for medical students and several general psychiatry residents. With over 25 years of psychiatry experience, he has developed numerous lectures including adolescent suicide, Attention Deficit Hyperactivity Disorder (ADHD), pervasive developmental disabilities, autism (including spectrum disorders) and child and adolescent affective disorders. He has presented papers at the American Psychiatric Association and the AACAP. He was the 1999 recipient of the American Psychiatric Association’s Nancy C.A. Roeske Certificate of Excellence in Medical Student Teaching.
Dr. Pahlavan is board certified through the American Board of Psychiatry and Neurology and the American Board of Child Psychiatry. He completed his general psychiatry residencies at Norwich Hospital, Connecticut; Medfield State Hospital, Massachusetts; and New England Medical Center in Boston. He completed his fellowship in child psychiatry at the McLean Hospital, Harvard Medical School in Massachusetts. He is a fellow of American Psychiatric Association and the American Academy of Child & Adolescent Psychiatry.
Bradley C. Riemann, PhD
Clinical Director, The Obsessive-Compulsive Disorder Center
Dr. Riemann is the clinical director of The Obsessive-Compulsive Disorder Center and cognitive-behavioral services of Rogers Memorial Hospital. Dr. Riemann is a leading expert in the assessment and cognitive-behavioral treatment of anxiety disorders and supervises the training program for graduate and post-graduate students from around the country for cognitive behavioral therapy in anxiety disorders.
Dr. Riemann is on the Scientific Advisory Board of the OC Foundation and the OC Foundation of Metropolitan Chicago as well as a member of the Publications Committee of the Anxiety Disorders Association of America. He is past president of the Board of Directors of the Mental Health Association in Waukesha County.
He has authored numerous scientific papers on obsessive-compulsive disorder and anxiety and has spoken at national and international conventions, including the Association for the Advancement for Behavioral Therapy, the American Anxiety Disorder Association and the Obsessive-Compulsive Foundation. He collaborates with colleges and universities all over the country on research projects investigating obsessive-compulsive disorder and anxiety. Dr. Riemann has also been featured on the national television shows 48 Hours and The Today Show.
Dr. Riemann received his doctorate in clinical psychology from the Chicago Medical School. He is also a clinical assistant professor in the department of psychology at the Chicago Medical School, Marquette University, and the University of Wisconsin-Milwaukee.

Jeffrey J. Smarrella, MD
Child and Adolescent Program : Oconomowoc Campus
Jeffrey J. Smarrella, MD, a child and adolescent psychiatry resident at University of Wisconsin Hospitals and Clinics in Madison, WI, is scheduled to join the Rogers Memorial Oconomowoc medical staff in August. He will be a full-time physician for the inpatient child and adolescent program. Dr. Smarrella received his medical degree from the Ohio State University College of Medicine in Columbus, OH, and completed his adult psychiatry residency at Dartmouth-Hitchcock Medical Center in Lebanon, NH.
Ted Weltzin, MD
Medical Director, Eating Disorder Services
Dr. Weltzin is a member of the American Psychiatric Association. In his most recent previous position, he was assistant clinical professor of psychiatry at the University of Wisconsin Medical School in Madison. From 1988 to 1994, he was assistant professor of psychiatry and medical director of the Center for Overcoming Problem Eating, an inpatient program at the University of Pittsburgh. He then was the director of the Depression Treatment Program at the University of Wisconsin in 1994 and 1995.
Dr. Weltzin has researched factors that contribute to menstrual irregularities, characterizing nutritional aspects of recovery from bulimia and anorexia, and identifying predictors of recovery from anorexia. He also researched the relationship between eating disorders and impulsivity and obsessive-compulsive disorder. He has lectured internationally and published over thirty research articles on a variety of eating disorder topics. He was an invited contributor to the initial American Psychiatric Association’s Eating Disorders Guidelines.
Nicolette E. Weisensel, M.D.
Eating Disorder Services
Dr. Weisensel is a board certified psychiatrist and a full-time medical staff member of the eating disorder services at Rogers Memorial Hospital in Oconomowoc, WI. Dr. Weisensel graduated in 2000 from the University of Wisconsin Medical School. She completed a four year general psychaitry residency at the University of Wisconsin Hospital and Clinics, serving as chief resident during her fourth year. She completed a three-month rotation during her fourth year of residency at the Eating Disorder Center at Rogers Memorial. Prior to joining Rogers Memorial, she was a resident physician at the University of Wisconsin Medical School in the Department of Psychiatry. She specializes in the treatment of eating disorders.
Dr. Weisensel is a past recipient of the American Psychiatric Association Aventis Travel Fellowship and a current member of the American Psychiatric Association and the Wisconsin Psychiatric Association.
